I saw one post about this problem (or a very similar one), but not much else.
I'm using Oblvion + Shivering Isles with the latest patch (v1.2.0416) without any mods.
I'm at the point where I've gathered the Elder Scroll and made my way back to Ocato's bedroom and I'm ready to jump down the chimney chute. I click on the chimney grate door icon and I get the completion message "I have escaped from the Imperial Palace by jumping down a chimney chute in Ocato's bedroom. ..." but I'm still in Ocato's bedroom! Yeesh! I tried reloading and removing various items but none of that made any difference.
The way I finally got out was to pickpocket the sleeping guards (I took there key's) until one caught me, and then payed the fine. I was then able to complete the entire quest and get the Gray Cowl.
Another odd thing, that's probably unrelated, is that when I was released a "bleak mine's guard" was trying to attack me in the city. I just watched as he was taken down by the city watch. I remember breezing through the Bleak Mine's quest without having to fight any guards, but they've shown up from time to time in Anvil and fought the guards there.
One other glitch (just in case it matters) is that I had the quest item the "Akaviri Orders" stuck in my inventory after stealing them once I finished the "Lifting the Vale" quest. I can try consoling those out of my inventory to see if it makes any difference.
Anyway, sorry for the long post, I just thought I'd report this. It was kind of a show stopper for me. I've completed a lot of quests so I suspect it's some kind of conflict.

Bleak Mine for me too
I was entereing the "Silverhome on the water" in bravil, as I was going to get some info about the Forlorn watchman. Then I saw two mine gurads standing up from the table and started charging at me. Poor [&@%!]
I used to play Oblivion, then I took an arrow in the knee.

Yeah, those attacks always throw me off. I'm always worried I'm going to accidentally hit a non-combatant entering into the fray.
I've collected a bunch of keys off of fallen agents, I'm wondering if I can find their houses and snoop around (not that I couldn't just pick the locks before, but now they should be empty).
