Odd tutorial glitch i saw

Basically, i have a weird fear of game models looking wrong. Missing features, like, for example, heads, and such. Thats probably why it caught me so off guard.

Anyway, i made a new save to mess around with the console. Of course, first thing i did was try to save everyone in the tutorial. It was going well until it came time for the emperor getting assasinated. I killed every assasin using the kill command, except, of course, it didn't really work out. At one point i looked at a pillar behind me, and noticed something on the floor right next to it, which had the name of the assasins, so i thought one of them simply glitched into the floor.
Well, maybe one of them did, but after my eyes actually focused on it, it looked like eyes and a mouth.
Im assuming me killing either the asassin responsible for killing the emperor, or one of the Blades caused his eyes and mouth to pop out or something. I could have also seen this wrong, since i turned away pretty fast, because of my dumb fear.
Basically, my question is - has anyone else seen this while using commands?

seen similar things if fallout new vegas and fallout 4, maybe in skyrim, tho have not played oblivion enough to remember it happening

I've seen a lot of weird things with the console. Sounds like when an NPC gets stuck between the walls, you'll usually see this happen (easiest way to recreate it is to take a door and keep opening and closing it with them in the doorway).
