A man is going to forge a sword while a bandit attacks a kid. In one dimension, the man kills the bandit and saves the kid while in another dimension the kid dies and the man ignores him and forges a sword. By the end of the Dragon Break, the man saves the kid and has that same forged sword in his hand.

Another one: A Glitch in the Matrix! You see, the world is like a machine but just that it has magic instead of gears and like all systems, there are glitches.

Overall: The man saves the kid and begins to have a deja vu when he goes to make the sword. It is because the sword is already there.

You're not correct: the man saves the kid, has the same forged sword in his hand and the kid and the bandit die. That would be a proper Dragon Break.

A dragon break is exactly what it sounds like: the Time Dragon breaks. It's not quantum physics or time travel or a glitch in the matrix, it's a nonsensical metaphysics explanation to roll all possible readings of an Elder Scroll into a single timeline on the premise that Time is no longer a factor during that period.

The way I see it is: two different possibilities rolled into one. The phrase, "When the Brass Tower walks and time is reshaped" works in this case. Some "change your fate" thing perhaps.
