The in-universe explanation is "you can't". I guess it depends on the method you cure yourself with, because you can certainly regain it in some of the other games.

Immortal Blood calls the Volkihar paranoid: http://uesp.net/wiki/Lore:Immortal_Blood but medically, it's not paranoia, if there are people legitimately trying to kill you. At worst, it is being overly cautious or blaming the wrong people.

Why do you assume it's not actual paranoia? For a vampire clan to be singled out as being particularly "paranoid and cruel", there's probably some serious issues there.

Keeping in mind that most vampires are paranoid and cruel; this one would be even more so.

Because vampire hunters are a fairly common profession? I recall hearing that paranoia is believing people are out to get you, when they are not. Therefore, you cannot be paranoid if people really are out to get you.

It's not paranoia. It's simply bad prioritizing ("Who to kill first? I know— Kill everyone first!").
Baurus: Our job is to get the Emperor out of situations like this. Though I'll admit, things aren't going exactly to plan.

Casandra: At the moment, the deadliest weapon at my disposal is a goblin's head on a stick. I'd be worried if this was the plan.

Incorrect, while in popular usage we often say that someone who suspects that someone is aiming to do them harm without evidence is just being paranoid. Clinical paranoia exists regardless of external factors, it's attributing benign or coincidental happenings to malicious forethought. But then you haven't considered that because the whole purpose of these posts by you is to act as an apologist for fictional creatures in a video game.

If you admit that this entire conversation is about fictitious creatures, then why even bother posting in it? Also, https://www.youtube.com/watch?v=zD8e5YDiyyU you aren't paranoid if people are out to get you. For most of us, believing that we will be sniped by a crossbow is being paranoid. Not for a vampire.

Then again— if you had a legitimate reason to fear being set upon and killed by angry mobs then mental fitness is the least of your issues.

Also, to call vampires paranoid is basically the same as calling every military or political leader, every security guard (hi, FNAF), and everyone who hesitates to give out their phone number for fear of harassment by telemarketers paranoid. If you would still call any of these paranoid, then paranoia in these circumstances is necessary for staying alive (again, hi, FNAF) and doing your job correctly, when applicable.
