Ursula's Stats are pretty high, but I think you can still benefit what ever level you are from these fabulous recipes.
Feel free to share your favorites too.

I use a similar poison, but make it with Thistle instead of Salt. Gives you Damage Health + Slow + Ravage Stamina. Very useful.

Actually this creates a potion that does fortify and restore health in addition to making conjuration spells last longer. If you don't need the conjuration bonus you only need the blue mountain flower and the wheat.

And I too love and use the slow+damage health recipe. Actually even just a poison of slow is super useful(Deathbell + Salt Pile are the easiest to find ingredients)

I haven't done a lot with Alchemy, but I like Luna Moth Wing and Vampire Dust for Invisibility + Health Regen.

Hopefully, these are all accurate. The character in the < avatar < discovered these, but she's on vacation so I can't easily verify all of them.

Poisons

Paralyze + Lingering Damage Health
Spoiler:
Recipe 1
1. Imp Stool
2. Briar Heart
3. Orange Dartwing or Slaughterfish Egg

Recipe 2
1. Imp Stool
2. Canis Root
3. Mora Tipanella or Orange Dartwing or Slaughterfish Egg or Slaughterfish Scales
Damage Health + Lingering Damage Health
Spoiler:
1. Imp Stool
2. Mora Tipanella or Orange Dartwing or Slaughterfish Egg or Slaughterfish Scales
3. Anything with Damage Health that doesn't adversely interact with the reagents above.
Augmented Shock
Weakness to Shock + Damage Health or Magicka
Spoiler:
1. Void Salts (Damage Health, Weakness to Shock)
2. Hagraven Feathers (Damage Magicka, Weakness to Shock)
3. Depends on whether you want to damage Health or Magicka
An anti-mage poison with just two reagents:
Damage Magicka + Damage Magicka Regen
Spoiler:
Glow Dust
Hanging Moss
Another anti-mage poison (or good general purpose poison)
Damage Magicka Regen + Damage Health (+ Drain Stamina)
Spoiler:
1. Nightshade
2. Bear Claws, Blue Butterfly Wing, Blue Mountain Flower, Chicken's Egg*, Glow Dust, Hanging Moss, Spider Egg, Spriggan Sap
3. Anything with Damage Health that doesn't produce beneficial effects for the target

*Note that Nightshade + Chicken's Egg = Damage Magicka Regen + Lingering Damage Stamina, so you can add something with Damage Health to produce an all-around useful poison.
Slow + Damage Health
Spoiler:
1. Deathbell
2. River Betty
Hadn’t thought of adding Thistle to it before. Good tip, Evil-i
Potions

Resist All Elemental Magic
Spoiler:
1. Snowberries (Resist Fire & Frost & Shock)
2. Hawk Beak (Resist Frost & Shock)
3. Most reagents with Resist Fire (Bone Meal, Dragon’s Tongue, Elves Ear, Fire Salts, Fly Amanita, Mudcrab Chitin)
Regenerate Health and Magicka
Spoiler:
1. Garlic (Regenerate Health & Magicka)
2. Jazbay Grapes
3. Juniper Berries
*Some of the other reagents with Regenerate Health or Magicka have negative interactions with each other, but Jazbay Grapes and Juniper Berries produce a clean potion.

Wheat+Blue Mountain Flower is pretty much cookie-cutter healing potion, since you can grow both ingredients at home with HF and they're plentiful everywhere.

Red Mountain Flower+Jazbay+Dwarven Oil or Taproot makes a great magic booster. Fortify+regenerate+restore magicka, with two cheap and plentiful ingredients.

Nightshade+Glowing Mushroom. Simple, easy to get ingredients, and the outcome is just as simple potion that makes Destruction viable in boss fights.

Ectoplasm+Glowing Mushroom+Red Mountain Flower is even better version of the previous. Adds restore+fortify magicka to Destruction boost.

While rare, Human Flesh is handy for a Paralysis+Damage Health+Lingering Damage Health combo with Imp Stool and Slaughterfish ingredients.

A Human Heart goes a long way to crafting a Damage Magicka+Damage Magicka Regen+Damage Health when combined with Nightshade and a typical Damage Magicka ingredient.
Baurus: Our job is to get the Emperor out of situations like this. Though I'll admit, things aren't going exactly to plan.

Casandra: At the moment, the deadliest weapon at my disposal is a goblin's head on a stick. I'd be worried if this was the plan.
