My Dremora used to be a dude

At what point in the game did your Sanguine Rose staff begin summoning a female Dremora?

Maybe at the same point I began using the same mods you're using.
Baurus: Our job is to get the Emperor out of situations like this. Though I'll admit, things aren't going exactly to plan.

Casandra: At the moment, the deadliest weapon at my disposal is a goblin's head on a stick. I'd be worried if this was the plan.

I'm at level 33 right now. The Dremora isn't supposed to change until I'm at level 36 or something. Today, the thing was female half the time, and male the other half. I installed SkyMoMod (Monster Mod) the other day. Maybe it plays with the Sam Stick? I just got everything working (216 mods right now), so I don't want to remove the mod to see if the female version still shows up. She doesn't talk at all, so it could be a mod (with no voice as usual) that is causing it.

You probably also downloaded some perverted sexlab mod that gives DD cups to females, and the game gives you female dremora so you can enjoy it.

None of the SexLab mods I have alter the look of NPCs. I'm in a new game now. Will see if it happens again.
