This is kinda like the Logical Inconsistencies thread in the Skyrim subforum, but anyway, I was in the town of Ripmarket and all of the shops were apparently owned by Doctors, even the ones that I really don't think a doctorate exists for. Similarly, when I asked questions to people, I always choose the polite version first. Many people are offended by this, and when I choose blunt, they answer instantly. Some people like dirty talking, huh?

The speech choices are intended to appeal to the person's social class. If you're out talking to a pickpocket and start using fancy or overly-polite vocabulary, you'll look like a fool. Makes sense to me.
